HIP ATTACK-2

A clinical trial to determine if accelerated medical clearance and surgery (with a goal of surgery within 6 hours of hip fracture diagnosis) compared to standard care will reduce the risk of death and other major health complications for patients with a hip fracture and an acute heart injury.

Eligibility Criteria

INCLUSION CRITERIA – Must be YES to questions 1 - 4

  1. Age ≥45 years.
  2. Diagnosis of hip fracture during working hours with a low-energy mechanism requiring surgery.
  3. Troponin elevation (at least one troponin measurement above the upper limit of normal occurring from the time of hip fracture to the time of randomization); and
  4. Informed consent.

EXCLUSION CRITERIA – Must be NO to questions 1 – 7

  1. Taking a therapeutic - dose of an anticoagulant and accelerated surgery is not feasible, or there is no reversible agent available.
  2. Patients with peri-prosthetic fracture, open fracture or bilateral fractures.
  3. Patients requiring an emergency surgery for another reason (e.g., subdural hematoma).
  4. Patients with acute myocardial infarction clinically unstable, with a mechanical complication (i.e., acute papillary muscle rupture, ventricular septal defect), ST elevation myocardial infarction, or cardiogenic shock.
  5. Patients refusing consent; or
  6. Patients previously enrolled in HIP ATTACK-2.
