Working in partnership to provide the best possible care for your patients remains our priority at Yale Medicine (YM). We value our relationships with referring providers and appreciate your confidence in our physicians and clinical programs.
With this in mind, we are excited to introduce our Physician Liaison Program, designed to support two-way communication and strengthen collaborations between our 1,200 physicians, referring providers and their staff.
Our Liaisons are available to:
- Connect referring physicians with YM specialists
- Provide information and education on new programs and services
- Assist in the resolution of service-related issues
- Support care coordination by helping referring physicians obtain timely follow-up information on their patients
- Connect physicians to the Yale Center for Clinical Investigation, which provides support and services for conducting clinical research

Meet our Physician Liaisons
Loren A. Van Ostrand
Loren comes to Yale with over 11 years of experience in the health care field. In her former roles at Hartford HealthCare and Georgetown University Hospital she worked extensively in developing strategies to enhance physician relations and communication between physicians and the hospitals. She holds a Bachelor's degree in Communication Management from the University of Dayton in Ohio.
Loren's service area is: Hartford, Litchfield, New London, Tolland, and Windham counties.
Frank has been at the Yale School of Medicine since 2005. He has over 12 years of experience in business development and CME programming in the health care field. Frank is dedicated to delivering the highest quality service and educating referring healthcare professionals. He is a graduate of the University of Connecticut with a Bachelor of Science in Molecular and Cellular Biology.
Frank's service area is: New Haven, Fairfield and Middlesex counties.
